Our social workers/nurses reach out to provide personalised, family-based, community support for patients and their caregivers along different stages of the cancer continuum. Services include telephone counselling, home visits, hospital outreach support, integrative body-mind-spirit programmes, purchase of community services (i.e. Chinese medicine, counselling, dietary, physiotherapy, home help, escort service, etc.) Same social worker/nurse will follow up the family for 4-9 months depending on their situation. All services are free of charge.

Research Report

The “Walking Hand-in-Hand” project was developed and guided by the core operating values of patient empowerment, personalized care, emphasis on holistic well-being and family orientation, among others. Objectives were fourfold:

  1. Improve quality of life of service users and reduce suffering;
  2. Ensure continuity of care amid movement between hospitals and the community;
  3. Provide readily accessible information and community resources at all stages of illness;
  4. Provide cross-discipline care to meet holistic needs of service users.
