Self-referral Procedure

1. Receive enquiry
Upon receive the enquiry call, a nurse/social worker will be designated to contact with patient within 3-5 working days
2. Assessment
Social workers/nurses will carry out the assessment and provide telephone counseling or home visits will be carried out to ease the difficulties
3. Home visit or phone follow-up
To provide personalised, family-based, community support for patients and their caregivers

Professional Referral Procedure

Receive referral
Medical / health & social welfare professionals can fill in and send/fax the referral form to us after asking for patient’s consent for referral
Confirm of receiving the referral
Acknowledge referee within 2 working days
Assessment
A designated social workers/nurses will contact patient/caregiver within 3 working days for assessing client’s need. All information provided are confidential
Home Visit or phone follow-up
Social workers/nurses will contact patient/caregiver through phone contact/home visit for detail assessment of the need within 2 weeks and will provide counseling, information and support
Brief report to referee
2 weeks after initial contacted patient/caregiver, a brief report will be send to referee
Brief report until end of service
At every 2-3 months afterward, a brief report will send to referee for updating until the end of service to patient/caregiver
